The Maryland Transition Digital Portfolio supports disabled students of transition age (14 and older) with the implementation and documentation of work and community-based experiences to improve post-secondary outcomes in collaboration with Maryland State Department of Education Division of Early Intervention and Special Education Services. The MDTP ports the transition goals and activities from the active Maryland Online Individualized Education Program, allowing students to take the lead on documenting their transition experiences by showcasing and highlighting their successes.
